hereby recognize under my sign and seal that I will take full responsibility
for solving |

| all matters
recited in the following sentences, and |

| strictly
observe not to cause any trouble to your company, |

| Tsukuba
Circuit, and/or any other organizations that you belong to, |
|
| during and after my attendance at "7１th Tsukuba Driving Lesson" |
|
| which is scheduled to be held at Tsukuba Circuit on Sep. ７th, Tuesday,
2010. |
|
| 1.
I take full responsibility for solving all incidents including death, injury,
and/or |
|
| any other accidents which would
possibly be caused while I participate in |
|
| "Tsukuba Driving Lesson". |
|
| In this connection, I hereby
swear that I will not accuse, |
|
| pursue responsibility, and/or
claim compensation for damage to |
|
| any one of your company, the
clerks and employees concerned |
|
| (including the owner of the
course), and other participants |
|
| for whatever damage I and/or the
vehicle in question may suffer for any reason whatsoever. |
|
| This is also true for the case
where the accident may be caused by mistake or |
|
| any other reason by the
organizing party or those who concerned. |

|
| 2.
I swear that I will take
responsibility for compensation for whatever damage caused by |
|
| an accident to the circuit and
the organizing party, |
|
| if the accident has been caused
by my own fault. This is also true for the case where |
|
| the accident may be caused by
mistake or any other reason by the organizing |
|
| party or those who concerned. |

| 3.
I swear that I will order other members of the party to strictly keep every
rule |
|
| and safety guidelines. |
|
| I will take responsibility for
any accidents the members of the party |
|
| may cause and/or may suffer. |
|
|
| 4.
I swear that I will obey whatever instructions given by the organizing
party |
|
| and/or those who concerned
as long as such instructions are for |
|
| the risk avert and the
smooth progress of the event. |
